

The Age-Friendly Health Systems initiative is based on the 4Ms Framework, which guides care for older adults across all settings:
• What Matters – Knowing and aligning care with each older adult’s specific goals and care preferences.
• Medication – Using age-appropriate medications and avoiding those that can interfere with what matters or cause harm.
• Mentation – Preventing, identifying, treating and managing dementia, depression and delirium.
• Mobility – Ensuring older adults move safely every day to maintain function and independence.
This initiative underscores Montefiore Nyack Hospital’s ongoing commitment to innovation in geriatric care and to improving outcomes for older adults.
Montefiore Nyack Hospital is a 391-bed community acute care medical and surgical hospital founded in 1895 in Rockland County, New York. Its mission is to provide competent, innovative and accessible emergency and acute care services to the residents of Rockland County and surrounding areas. It is a member of Montefiore Health System and a partner with Albert Einstein College of Medicine and Touro College of Osteopathic Medicine, providing clinical rotations to medical students.
