A national movement, led by The John A. Hartford Foundation and the Institute for Healthcare Improvement, in partnership with the American Hospital Association and the Catholic Health Association of the United States, is helping health-care organizations implement a set of evidence-based interventions specifically designed to improve care for older adults. Collaborative learning opportunities across the movement bring together health-care teams committed to sharing data and improving outcomes. All teams strive toward implementing age-friendly best practices across hospitals, clinics, nursing homes, home health agencies and primary and specialty care settings.

The Age-Friendly Health Systems initiative is based on the 4Ms Framework, which guides care for older adults across all settings:
• What Matters – Knowing and aligning care with each older adult’s specific goals and care preferences.
• Medication – Using age-appropriate medications and avoiding those that can interfere with what matters or cause harm.
• Mentation – Preventing, identifying, treating and managing dementia, depression and delirium.
• Mobility – Ensuring older adults move safely every day to maintain function and independence.
This initiative underscores Montefiore Nyack Hospital’s ongoing commitment to innovation in geriatric care and to improving outcomes for older adults.
