White Plains Hospital is the only hospital in the United States awarded with the 2025 American Nurses Credentialing Center (ANCC) Magnet Prize®, sponsored by Press Ganey. The recognition was announced during the 2025 ANCC Magnet & Pathway Conference™ on Oct. 8 in Atlanta.
The prize is presented annually to one Magnet-designated organization whose nursing team best demonstrates exemplary innovation in patient care delivery, including notable achievements both within practice and in research that result in improved patient health outcomes.
White Plains Hospital was selected for its innovative project, “Revolutionizing Diabetes Transitional Care: A Nurse-Led Remote Patient Monitoring (RPM) Model to Advance Equity and Outcomes.” This award-winning project combines nurse-led care and cutting-edge technology to provide real-time clinical guidance to patients to help control their A1c levels.
Specifically, the White Plains Hospital RPM Model utilizes a 90-day transitional care model that incorporates continuous glucose monitoring, structured clinical coaching, and a pathway to improve outcomes for patients. Nurses access the continuous glucose monitoring data through a HIPAA-compliant dashboard to help inform structured education, behavioral coaching, medication review and psychosocial support to bolster stabilizing glycemic patterns and avoid complications.
