The Watermark at 3030 Park”™s rehabilitation and skilled nursing facility, The Springs, has been selected as a provider to the Yale New Haven Health post-acute network. The network, which includes neighboring Bridgeport Hospital, will offer services to all current residents at The Watermark as well as seniors living within the broader community.
Yale New Haven Health”™s development of the post-acute network is aimed at strengthening the hospital”™s post-acute connection by improving patient outcomes and decreasing lengths of stay and readmissions. In order to be admitted into the post-acute network, according to a press release, The Watermark has to meet specific criteria, including:
Ӣ Maintaining a quality rating of three stars or higher on Medicare Nursing Home Compare site;
Ӣ Working with Yale New Haven Health System to develop and utilize protocols for conditions such as congestive heart failure, chronic obstructive pulmonary disease, pneumonia and hip fractures;
”¢ Participating in the health system’s initiatives designed to improve hospital transitions that reduce readmission and transform care for each patient; and
Ӣ Ensure patients get the right care at the right time while avoiding unnecessary duplication of services and reducing medical errors.
Watermark Executive Director Kristin Butler noted that, in addition to short-term rehabilitation, the Bridgeport facility is licensed to provide independent living, assisted living, skilled care and memory care.
