MKMG, Empire BlueCross launch enhanced care program

Officials at Mount Kisco Medical Group, a multispecialty physician group practice, and Empire BlueCross BlueShield, the state”™s largest health insurer, recently announced the start of their new collaboration through Empire”™s Enhanced Personal Health Care Program.

The agreement between the regional health care provider and the insurer aims to improve the health of Empire plan members through coordinated care and by improving efficiencies that help to avoid duplicative services, unnecessary emergency room visits and preventable hospital admissions.

Mount Kisco Medical Group”™s care management team will develop individualized care plans for patients with chronic or complex conditions and continuously monitor patients”™ health according to tested guidelines for care and services. The care management team is trained to provide health education, care transition assistance and other services to all Empire members in addition to intensive case management.

The agreement covers approximately 22,000 members of Empire commercial plans.
