Greenwich Hospital to transition outpatient clinic services to Family Centers

Greenwich Hospital is partnering with Family Centers, a nonprofit organization offering education, health and human services, in a new initiative that will transition the hospital”™s Outpatient Center and clinic services to Family Centers.

Greenwich Hospital plans to file a Certificate of Need (CON) in June for approval of this transition. Subject to governmental approvals, Family Centers will assume management and day-to-day oversight of the clinics in a newly renovated space at 75 Holly Hill Lane, which is expected to open in 2024. Among the services being transitioned to Family Centers are the adult medicine clinic, including women”™s health and geriatrics, and the pediatric primary care clinics.

Until the new space opens, the hospital will continue to offer all services at its current clinic locations at 500 West Putnam Ave. and 5 Perryridge Road. As part of the collaboration, Family Centers will also adopt Epic, the electronic health record currently used by Greenwich Hospital.
