Montefiore Nyack Hospital is preparing the opening of its new transitional care unit (TCU), which is scheduled to begin accepting patients at the end of the month.
“The TCU gives patients a choice on how and where to receive extra care that will ready them to go home safely,” Montefiore Nyack Hospital President and CEO Mark Geller said. “Our multidisciplinary team of rehabilitation professionals create a treatment plan to restore maximum functioning in the shortest period of time. With almost three hours of physical therapy a day, patients at the TCU receive more rehabilitation than outpatient therapy can provide, expediting their recovery and return home. We also provide comprehensive patient and family education all of which is designed to promote wellness and eliminate an unnecessary need for emergent care.”
